Echelon Group, LLC dba CentricHC (“CentricHC”, “we” or “us”) is committed to protecting your privacy on our websites (collectively, “Websites”). This policy only applies to our Websites and the information that we collect through our direct interactions with you.  We provide this Privacy Notice to inform you about the type of personal information we collect from you online or offline and how we use, share, and protect that information. For purposes of this Notice, “personal information,” is any information that identifies or relates to an identified or identifiable natural person.’

This privacy notice applies to our collection of personal information as a controller of the data. This privacy notice does not apply to personal information that is subject to the Health Insurance Portability and Accountability Act (“HIPAA”) that we process as a “business associate” on behalf of your employer-sponsored health plan or plan administrator. When we process information as a business associate, your health plan's or plan administrator’s privacy policies apply to that information. TYPES OF PERSONAL INFORMATION WE COLLECT

Information You Provide to Us 

When you interact with us, you may provide us with personal information such as your name, email, telephone number, address, employer, demographic information (such as age or gender), payment information, and financial information. We use this information to provide you with information, improve our services, and to communicate with you. 

Information We Collect Automatically

Whenever you visit or interact with our Websites, we may use a variety of technologies to collect information about your usage. We may collect your IP address, device identifier, browsing or search history, preference information, account settings (including any default preferences), and other information regarding your interactions with and use of our Websites. We may also collect your non-precise geolocation derived from your IP address. We use this information to analyze how users interact with our Websites and improve the user experience on our Websites. HOW WE USE YOUR PERSONAL INFORMATION

We may use your Personal Information in the following ways:

  1. To provide you with our services, such as providing and delivering you the goods, services and information you request.
  2. For our internal business purposes, such as operating our Websites and customizing the content; maintaining internal business records; enforcing our policies and rules; IT administration of our network, intranet, and other technologies; and IT security management and tasks.

HOW WE DISCLOSE PERSONAL INFORMATION

We do not sell your personal information.  In addition to disclosures made with your consent, we may disclose your personal information to certain third parties as described below:

  1. Affiliates, members of our group company and subsidiaries, including parent entities, corporate affiliates, subsidiaries, business units, and other companies that share common ownership for business functions.
  2. Service providers that work on our behalf to support our relationship with you, such as IT providers, internet service providers, web hosting providers, data analytics providers, and companies that provide business support services, financial administration, or event organization.
  3. Professional consultants, including accountants, lawyers, financial advisors, and audit firms for legal and accounting purposes.
  4. Law enforcement, government agencies, to comply with law or legal requirements for legal, security, or safety purposes.
  5. Other entities in connection with a corporate transaction, if we, or some or all of our assets, are acquired by another entity, including through a sale in connection with bankruptcy or other forms of corporate change.
  6. Business partners that may use Personal Information for their own purposes, such as companies that operate cookies and other tracking technologies, social media companies, data brokers, and other business partners for their own marketing, research, or analytics purposes. Where required by law, we will obtain your consent prior to disclosing your Personal Information to our business partners.

COOKIES, AND OTHER TRACKING TECHNOLOGIES

Types of Tracking Technologies We Use

  • Cookies, which are short text files may be downloaded to your computer during the course of your visit to our Websites. You are not obliged to accept a cookie that we send to you, and you can in fact modify your browser so that it will not accept cookies.  Our website uses Google Analytics to help us analyze how users use the website and target users with appropriate advertisements. The information generated by the cookie about your use of the website (including your IP address) will be transmitted to and stored by Google. Read about Google's privacy practices
  • Pixels, web beacons, and tags, which are types of code or transparent graphics that contain a unique identifier. These technologies provide analytical information about the user experience and help us customize our marketing activities. In contrast to cookies, which are stored on a user's computer hard drive, pixels, web beacons, and tags are embedded invisibly on web pages.

Cookie management: You have the ability to accept or decline cookies. Most browsers automatically accept cookies, but you can usually modify your browser setting to decline cookies if you prefer. If you choose to decline cookies, you may not be able to fully experience the interactive features of the Websites.

SECURITY OF INFORMATION

We have implemented reasonable administrative, technical, organizational and physical measures designed to prevent unauthorized access, improper use or disclosure, unauthorized modification or unlawful destruction or accidental loss of personal data.  However, no transmission over the Internet or any network can be 100% secure.  Transmission of personal information to CentricHC over the internet is at your own risk.

DO NOT TRACK SIGNALS

Web browsers and other technologies you may use to access Websites may include a Do-Not-Track (“DNT”) feature or setting you can activate to signal your privacy preference not to have data about your online browsing activities monitored and collected. We do not currently respond to DNT browser signals or any other mechanism that automatically communicates your choice not to be tracked online.

CHILDREN’S PRIVACY

Our Websites are not directed to children under 18 years of age, and we do not knowingly collect any personal information of children under 18 years of age. If you believe a child under 18 years of age has provided us with personal information, please contact us at CARE@centrichc.com.

NOTICE TO CALIFORNIA RESIDENTS

California Civil Code Section 1798.83, also known as the “Shine The Light” law, permits our Website visitors who are California residents to request and obtain from us, once a year and free of charge, information about categories of personal information (if any) we disclosed to third parties for direct marketing purposes and the names and addresses of all third parties with which we shared personal information in the immediately preceding calendar year. If you are a California resident and would like to make such a request, please submit your request in writing to us using the contact information provided below. 

At this time, we are not subject to the California Consumer Privacy Act, but if we become subject to that law, we will update this privacy notice. 

NOTICE TO NEBRASKA AND TEXAS RESIDENTS
You may have the following rights, subject to any applicable exemptions or limitations:

  • Right to Access. You have the right to confirm whether we are processing your personal information and to access such personal information.
  • Right to Correct. You have the right to request that we correct inaccurate personal information that we maintain about you.
  • Right to Delete. You have the right to request that we delete your personal information under specific circumstances.
  • Right to Opt-out. You have the right to object or opt-out of certain types of processing, including: (1) processing for the purpose of targeted advertising, (2) processing for the purpose of the sale of personal information, and (3) processing for the purpose of certain types of profiling and automated decision-making.
  • Right to Data Portability. You have the right to request a copy of your personal information in an accessible format.
  • Right to Equal Service. If you choose to exercise any of these rights, we will not discriminate against you in any way.  If you exercise certain rights, understand that you may be unable to use or access certain features of our websites, services, or properties.
  • Right to Appeal. If we do not grant your consumer request, you may have the right to appeal that denial.

Making a consumer request does not require you to create an account with us. Applicable law may limit the number of times that you can submit a particular request within a 12-month period.

You can designate an authorized agent to make a request on your behalf. We may deny a request from an authorized agent that does not submit proof that they have been validly authorized to act on your behalf in accordance with applicable law.

Upon receiving your request, we will need to verify your identity to determine you are the same person about whom we have the information in our system. These verification efforts require us to ask you to provide information so that we can match it with information you have previously provided us. For instance, depending on the type of request you submit, we may ask you to provide certain information so that we can match the information you provide with the information we already have on file, or we may contact you through a communication method (e.g. phone or email) that you have previously provided to us. We may also use other verification methods as the circumstances dictate. 

We will only use personal information provided in your request to verify your identity or authority to make the request. To the extent possible, we will avoid requesting additional information from you for the purposes of verification.  If, however, we cannot verify your identity from the information already maintained by us, we may request that you provide additional information for the purposes of verifying your identity, and for security or fraud-prevention purposes.  We will delete such additionally provided information as soon as we finish verifying you.

NOTICE TO INTERNATIONAL USERS

CentricHC is located in the United States, and we recognize that the laws in the United States may be different and, in some cases, less protective than the laws other countries, including the European Economic Area (“EEA”). By providing us with your personal information and using our Websites, you acknowledge that your personal information will be transferred to and processed in the United States.

LINKS TO THIRD PARTY WEBSITES

While browsing our Websites, you may encounter and choose to access websites or online services operated by third parties by clicking on hypertext links or icons. These websites may collect data or personal information about you and your online activities. We do not control and are not responsible for what these other parties do in connection with their websites or online services, or how they handle your personal information.

From time to time, we may make changes to this Privacy Notice. Any changes will be effective when posted. Please check this Privacy Notice for updates periodically. When required by law, we will attempt to notify you with updates to this Privacy Notice.
